Job Description:
Job Description Job Description The Case Manager is responsible
for coordinating hospital’s Case Management for patients with the
goal of providing effective discharge planning to reduce
recidivism. Assesses, plans, implements, coordinates, monitors, and
evaluates options to facilitate the continuum of care including
follow-up appointments, medication/prescription management,
housing, and transportation arrangements upon discharge. Duties
include but are not limited to: Access to daily admission report
with purpose of effectively meeting new patients to coordinate
discharge planning. Meet with new patients/ contact patient
guardian in first 24 hours of admission to complete required
activities i.e. obtain collateral information regarding living
arrangements, set up family therapy session, patient request and MD
ordered activities. Educates patients regarding effective
allocation of community mental health resources while encouraging
provision of high quality patient care. Document appropriately
gained legal authority to communicate with family members to gain
collateral information through ROI. Assess for consent for
treatment documentation; voluntary/involuntary status of patients
and obtain consent for treatment from patient or patient’s guardian
if no consent for treatment exists on file. Notify Lead Therapist
of any patients needing to be court ordered due to inability to
obtain consent for treatment. Update case management board with
correlating information obtained from collateral call/patient
interview. Ability to identify and communicate with Lead Manager
regarding any high-risk patients and discuss alternative
interventions needed for safe discharge to community. Become aware
of projected length of stay based on clinical and insurance
coverage. Complete insurance bridges as appropriate and communicate
with insurance case managers regarding updates, incidences, and
discharge planning. Attend Pre-Flash and Treatment Team meetings
and all meetings relative to case management/ social functions.
Documents clearly and concisely all required documentation i.e.
contacts and information of the patient’s case management process
on the appropriate chart forms. Maintains currant knowledge of case
management/socials services, discharge planning, and stay abreast
of hospital programs and services provided. Attends approved
continuing education training regarding mental health services.
Suggests and introduces new methods and techniques to leadership.
Develops and maintains therapeutic relationships with clients
serving as a role model for clients by demonstrating positive,
proactive, and respectful communications. Provides one to one and
group support and skills training, individualized interventions and
crisis management using a client centered approach. Ensures
individualized treatment strategies and placement is consistent
with client's needs and goals. Regularly participates as a member
of an inter-disciplinary team providing consultation and client
services. Ensures patient receives the most appropriate level of
care and treatment recommendation. Upholds the Organization's
ethics and customer service standards. Requirements Education:
Degree in field of Social Services or BSW preferred. Bi-lingual
English/Spanish preferred Licenses: Must obtain and maintain
approved CPR certification as well as successful completion of
Management of Assaultive Behavior training prior to assuming
patient care responsibility.? Valid Texas counselor or nursing
license a preferred. Experience: Two (2) years’ experience in a
psychiatric health care facility, direct practice in mental health
treatment. Experience in crisis intervention and customer service
preferred. Bi-lingual English/Spanish a plus Knowledge, Skills &
Abilities: Knowledgeable of patient rights and laws (i.e. EMTALA)
pertaining to mental health and regulations pertaining to
psychiatric nursing and the treatment of the mentally and
emotionally disturbed patients. Theories, principles, and
techniques of group and individual psychotherapy. Displays basic
knowledge of treatment procedures; interventions common to acute
psychotic as well as non-violent crises intervention practice,
medical, psychiatric emergency procedures and crisis intervention
skills. Communicate effectively with a variety of individuals and
function calmly in situations, which require a high degree of
sensitivity, tact, and diplomacy. Ability to exercise appropriate
judgment in answering questions and releasing information; analyze
and project consequences of decisions and/or recommendations.
Ability to handle confidential information with great sensitivity.
Well-developed verbal and written communication skills in English.
Skill to communicate effectively in order to exchange and/or
provide information, build and/or maintain cooperative
relationships. Displays basic knowledge of treatment procedures;
interventions common to acute psychotic as well as non-violent
crises intervention practice. Skilled in problem solving,
prioritizing and time management. Ability to document Case
Management plans in a clear and concise manner. Knowledge of state
and federal programs and regulations that govern hospital discharge
planning and those providing medical care and financial support to
individuals, community resources. Screening: Must successfully pass
background check, drug screen, physical and be able to provide
positive employment references. Physical Demands: With or without

and move objects from position to position (up to 50 lbs), finger
grasp; feel sizes, shapes, temperatures, and textures; express or

quickly; visually detect, determine, perceive, identify, recognize,
judge, observe, inspect, assess; perceive the nature of sound with
or without correction; perform repetitive motions of the wrist,
