Job Description:
Job Title: Diabetes Educator Profession: Healthcare Specialty:

Shift: 08:00 - 17:00 Experience: 0 - 2 years of related experience
License: Basic Life Support (BLS) required Certifications:
Registered Dietician (RD) preferred; Certified Diabetes Care and
Education Specialist (CDCES) preferred Must-Have: Bachelor's Degree
in nutrition or a related field Description: Responsible for
diabetes instruction, documentation, and follow-up on patients
identified through physician referral. Conducts quality and
effective diabetes self-management education program classes.
Accountable for ongoing evaluation of the program, performance
improvements, and physician and patient communication. Primarily
responsible for outpatient appointments. Develops, implements, and
evaluates community educational events. Participates in community
diabetes screenings. Maintains recognition program status.
Responsibilities: Provides outpatient diabetes self-management and
nutritional instruction consistent with program content and
guidelines. Conducts self-management classes conducive to learning
with satisfactory evaluations. Performs patient diabetes
instruction per physician orders. Instructs according to
age-specific guidelines and medical conditions. Provides family
support and education as needed. Refers to self-management classes
when appropriate. Documents and evaluates the teaching and learning
process in the patient’s chart. Provides professional and timely
information regarding patient progress and status. Maintains and
supports recognition program status. Develops, implements, and
participates in community diabetes and educational programs.
Evaluates the effectiveness of community outreach events. Assists
with diabetes education in-service for nursing staff and
physicians. Participates in multidisciplinary meetings with
committees and other healthcare professionals involved in the care
of diabetes patients. Represents and promotes the organization.
Provides input and assists with the planning of the center’s
services. Facilitates participant access to services. Strives for
the highest standards of productivity and quality instruction.
